MelZenith operates www.melzenith.com. This policy describes information handled through the website and its contact and appointment forms.
Contact enquiries
When you submit the contact form, we collect your name, email address, message and information about how you found MelZenith. Accepted enquiries are saved so the practice can review and respond to them. Submitted details are also included in email notifications to the practice’s designated recipients.
Appointments
The booking form collects your name, email address, callback phone number, reason for booking and selected appointment time. We use your email address to send a verification code before confirming an appointment. Confirmed bookings are saved. We use email to notify the practice and send you appointment details, a Zoom joining link when available, a link to change or cancel your appointment, and a calendar attachment.
Services used by this website
Cloudflare provides website hosting, data storage, email delivery, checks against automated submissions, and website usage and performance analytics. These services process information needed to deliver and protect the website, including technical request information and your IP address for form verification. Contact and booking submissions are also checked against spam and abuse rules. See Cloudflare’s privacy policy and its Turnstile privacy information.
Online meetings and calendars
Zoom provides online Discovery Calls. To create your meeting, we send Zoom a meeting title containing your name, the scheduled time and duration. See Zoom’s privacy information. Appointment details can also be included in calendar files sent to you and the practice.
Cookies and stored information
Essential session cookies are used for authorized access. Enquiries, confirmed appointment details, and related verification and delivery records are stored to operate these services. Copies of contact or appointment information may also be present in notification emails and calendar entries.
Your information and requests
You can request an exported copy of the personal data we hold about you, including information you have provided. You can also request that we erase your personal data.
